Michael Mulligan 5d978bb6e5 CaseInsensitiveArray rewrite for performance. 
Storeing the Case-Insensitive/Sensitive Keys separately increases performance.  Also, fix for unset and valid, as well as the ability to initialize with a pre-existing Array.
